The RiseBoro Case Management Program provides homebound seniors with comprehensive strength-based intakes and in-home assessments. We provide essential assessment and referral resources for home care and home delivered meals as well as assistance with entitlements and benefits. Ongoing case management and care planning address other unmet needs. Homebound seniors (or their caregivers) in need of assistance are provided information and referral, assessment for entitlements and benefits, home-delivered meals, home care and other services enabling them to stay in their homes. Case managers refer clients to Medicaid services if they meet the eligibility requirements. Seniors who are above the Medicaid income threshold receive ongoing case management assistance. If these clients are in need of homebound meals or home care, or other services, case managers make referrals to and coordinate services with provider agencies.

Our staff consist of professionally trained Case Managers who are supervised by credentialed MSW’s (Master’s in Social Work). Our case managers have vast experience in the field of aging and case management. They provide bilingual services including: Spanish,Polish, Russian and Yiddish. We also have access to translation services for clients with other language needs.

Currently, in order to receive services via RiseBoro Case Management, participants must be 60 years of age or older, homebound, and live in the neighborhoods of Bushwick, Greenpoint, Williamsburg or Bedford Stuyvesant.

Our clients can be linked to our Home Delivered Meals Program, DFTA Funded Home Care and Housekeeping Services. A referral for case management services can be made via phone and anyone can make a referral.
