Building a thriving healthy community

RiseBoro Health provides vital care for our seniors and community members at home and in our facilities. In 2001, we established the Buena Vida Health Care Center. Now home to more than 240 long-term and short-term residents, our dedicated bilingual staff provide continuing care and rehabilitation to our aging and disabled populations. Outside of the center, our staff visit homes, knock on doors and go beyond expectations every day to make sure community members get the care they need. Our recent partnership with NYU Langone Brooklyn PPS to provide FREE Asthma Home Assessment(s) and the OneCity Health Innovation Grant–RiseBoro was awarded the largest grant to help Brooklyn residents address above-average levels of obesity, diabetes, and uncontrolled hypertension. These community health programs are instrumental to building a thriving healthy community.

Assisted Living Program

Moffat Gardens is an Assisted Living Program (ALP) offering 26 independent living apartments within a HUD Section 202 senior citizen building. Residents live in their own apartments and are given the ongoing support to remain active in the community.
